Overview

We are seeking a dedicated Primary Care Coordinator to join our team at the Adams Lake Indian Band, collaborating closely within an interdisciplinary healthcare team at the Sexqeltqin Health Clinic. Though this is not a position within the nursing field; this role is crucial in providing holistic and culturally sensitive healthcare services, promoting health and well-being.

The Primary Care Coordinator supports the primary care providers and the allied health team by coordinating client care through a wrap-around approach. This involves collaborating with various team members to complete essential tasks such as PWD forms, treatment center applications, subsidy forms, and navigating complex health and social systems. This role ensures that all aspects of patient care are seamlessly integrated and efficiently managed.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

  • Performs all duties and responsibilities in accordance with Adams Lake Indian Band policies and procedures
  • Client Support: Provide individual and group counselling to community members, addressing mental health, family dynamics, and social issues
  • Health Coordination: Collaborate with healthcare providers to ensure comprehensive care plans that respect cultural practices and values
  • Advocacy: Advocate for clients’ needs within the healthcare system, ensuring their voices are heard and respected
  • Community Outreach: Develop and implement outreach programs tailored to community needs, including workshops and support groups
  • Resource Management: Connect clients with relevant resources and services, such as housing, financial assistance, and educational programs
  • Crisis Intervention: Provide immediate support and intervention during times of crisis, ensuring safety and well-being
  • Performs other duties as required

QUALIFICATIONS, KNOWLEDGE, AND SKILLS

  • Bachelor’s or Master’s degree in Social Work (BSW/MSW) or a Registered Nurse with a background in care coordination, care navigation, and primary care
  • Must be registered with the appropriate social work regulatory body
  • Previous experience working with Indigenous communities preferred
  • Familiarity with the history of colonization and its impacts on health and well-being
  • Experience working as part of an interdisciplinary team and primary care clinic
  • Strong communication, empathy, and problem-solving skills
  • Ability to work collaboratively with multidisciplinary teams
  • Deep understanding and respect for Indigenous cultures, traditions, and values
  • Respectful and appreciative of cultural diversity
  • Committed to the well-being of the community and proactive in addressing unique needs
  • Flexible and able to navigate complex and dynamic environments
  • Upholds ethical standards and confidentiality in all interactions

WORKING CONDITIONS

Most work is performed in a typical office environment; some work will be performed at scheduled events and gatherings. Travel may be required for events and community functions. Occasionally stressful situations.

Current Criminal Records Check with vulnerable sector clearance, with results acceptable to ALIB

TERMS OF EMPLOYMENT:

  • Full time permanent position
  • Wage range: $35 – $45 per hour
  • Upon completion of probation ALIB offers comprehensive Benefits and a Pension Plan.
  • Hours per week: 75 hours biweekly (max 37.5 hrs./week), with flexibility in hours/days of work.
