Home and Community Health Nurse

The Lytton First Nation (LFN) is recruiting a qualified and experienced Full-Time Home and Community Nurse for the Tl’Kemtsin Health Office. The Home and Community Health Nurse will be responsible to plan, organize, implement, and evaluate professional nursing care for clients with acute, chronic or palliative health care needs and their caregivers in a clinic, client’s home and/or community setting. Care is family centered and emphasizes the promotion, maintenance and restoration of health, the prevention and treatment of disease through teaching, counselling and direct nursing care and the provision of support for the client with a terminal illness, caregivers and other health care providers. The Home and Community Nurse participates in health promotion activities and acts a liaison between the program, hospital units, community health providers and community support services.

Principal Duties and Responsibilities:

• Provide nursing services to clients in their homes, community, or clinical setting

• Work within the context of the client-centered, family focused vison and goals of Lytton First Nation Tl’Kemtsin Health Center, FNHA Home and Community Care Program, and in accordance with the British Columbia College of Nurses and Midwives (BCCNM)

• Assist with the triage of client referrals, in accordance with establish guidelines, practice standards and criteria

• Follow up with clients and/or families, physicians, and other health care providers regarding complex medical conditions through client reassessments and the sharing of medical information

• Participate in the ongoing planning, development and evaluation of the Home and Community Care Program (within Interior Health Authority, First Nations Health Authority and other care providing agencies)

• Provide clients and family caregivers with health information as required through demonstrations and informal teaching sessions • Maintain appropriate client records, ensures confidentiality of all information relating to the client, and submits statistics and reports as required on a timely basis

• Perform other related duties as assigned

• Ensure cultural safety at all times with the provision of care

Qualifications and Requirements:

• Bachelor’s degree in nursing with 2-3 years experience in Home and Community Care (experience in public health or community nursing an asset)

• Current practicing Registered Nurse (RN) as a member in good standing with the British Columbia College of Nurses and Midwives (BCCNM) or  Current practicing Licensed Practical Nurse (LPN) with applicable experience working in First Nation communities.

• Comprehensive knowledge of Aboriginal Health Programs, as well possess a broad knowledge of Federal and BC’s health care systems

• Current CPR and First Aid Training

• Ability to guide Health Care Aide’s and ability to establish realistic performance expectations

• Must have and maintain a valid BC driver’s license as a condition of employment and have access to a reliable vehicle

Skills and Competencies:

• Ability to work independently, with minimal supervision

• Excellent verbal and written communication skills and proven ability to establish rapport with people of all educational and occupational backgrounds

• Experience working with First Nations people and communities and an understanding of historical impact issues and long-term trauma effects on First Nations people, specifically with residential school impacts (Intergenerational trauma)

• Ability to communicate effectively with First Nation’s people, families, and service providers
